Marijuana and Gabapentin: Can You Take Them Together?

Gabapentin and marijuana are sometimes used by the same patients for remarkably similar reasons. Gabapentin is frequently prescribed for nerve pain and certain seizure disorders, while medical cannabis is commonly used by people seeking relief from chronic pain, neuropathy, sleep problems and other persistent symptoms. That overlap raises an obvious question: Is it safe to use marijuana while taking gabapentin?

There is no well-established chemical interaction showing that marijuana makes gabapentin dangerously accumulate in the bloodstream, and cannabis is not specifically listed as a contraindication in standard gabapentin labeling. That does not, however, mean the combination has been proven harmless. Gabapentin and THC can both cause sleepiness, dizziness, impaired coordination and slowed psychomotor performance. A small controlled human study directly combining gabapentin with THC found evidence that their subjective and performance-impairing effects can overlap and become greater when used together. For most patients, therefore, the primary concern is not a classic liver-enzyme drug interaction but additive effects on the brain and central nervous system.

What Gabapentin Does

Gabapentin is an anticonvulsant medication originally developed for epilepsy. In the United States, FDA-approved formulations are used for indications including partial-onset seizures and postherpetic neuralgia, the nerve pain that may remain after shingles. Gabapentin is also widely prescribed off-label for other forms of neuropathic pain. The UK’s NICE neuropathic-pain guideline, for example, lists gabapentin alongside amitriptyline, duloxetine and pregabalin as initial treatment options for neuropathic pain other than trigeminal neuralgia.

Despite its name, gabapentin does not simply act like the neurotransmitter GABA. Its important neurological effects involve binding to the alpha-2-delta subunit of voltage-gated calcium channels, changing neurotransmitter release and reducing abnormal neuronal signaling. It also has an unusual pharmacokinetic profile compared with many prescription drugs. Gabapentin is not appreciably metabolized by the liver and is eliminated largely unchanged through the kidneys. University of Michigan researchers reviewing its pharmacokinetics noted that it does not induce hepatic enzymes and is primarily cleared through renal excretion. This becomes important when considering marijuana because many cannabinoid drug interactions involve liver enzymes, whereas gabapentin largely bypasses that pathway.

Is There a Direct Marijuana-Gabapentin Drug Interaction?

Current evidence does not demonstrate a major pharmacokinetic interaction in which THC or CBD reliably raises gabapentin concentrations by blocking its metabolism. Because gabapentin is not significantly metabolized through the cytochrome P450 liver-enzyme system and is instead excreted unchanged in urine, the type of CYP-mediated interaction associated with CBD and some other medications is less biologically likely with gabapentin. That distinction is useful: the concern is generally not that cannabis suddenly doubles the amount of gabapentin in the bloodstream.

The more plausible interaction is pharmacodynamic, meaning both substances can affect some of the same functions even though they reach those effects through different mechanisms. Gabapentin commonly causes somnolence, dizziness and ataxia. In controlled epilepsy trials cited in the Neurontin prescribing information, somnolence occurred in 19% of gabapentin-treated patients versus 9% receiving placebo, dizziness in 17% versus 7%, and ataxia in 13% versus 6%. Cannabis can independently impair coordination, reaction time, memory, judgment and decision-making. The CDC consequently advises against driving after cannabis use because these effects can interfere with safe vehicle operation. Combining two substances capable of producing dizziness and motor impairment may make those effects more noticeable.

What the Human THC and Gabapentin Study Found

One of the most relevant experiments was conducted by researchers at the University of Kentucky and published in Behavioural Pharmacology. Eight regular cannabis users were given oral THC at doses ranging from 5 to 30 mg, gabapentin at 600 or 1,200 mg, or combinations of the two. Investigators evaluated subjective drug effects, psychomotor performance, heart rate, blood pressure and participants’ ability to distinguish THC from placebo. Gabapentin itself produced subjective and performance effects that overlapped with THC, and when the drugs were administered together, the combinations generally produced larger effects on THC-sensitive measures than THC alone.

The study should not be interpreted as proof that normal medical marijuana use with prescribed gabapentin is inherently dangerous. It included only eight participants, used controlled doses of oral THC and was designed to investigate drug-discrimination effects rather than the treatment of chronic pain. Still, it provides direct human evidence supporting what clinicians might predict from the medications’ side-effect profiles: gabapentin can intensify some of the functional effects experienced with THC. A separate animal study from the University of Sydney also found interaction between THC and gabapentin in a neuropathic-pain model, including enhanced analgesic effects, but animal findings cannot establish an appropriate or safe human combination.

Dizziness, Sedation and Falls May Be the Most Common Problems

For many patients, the biggest practical issue is excessive sleepiness or dizziness. Someone who takes gabapentin at night and then uses a high-THC edible may feel considerably more sedated than expected. Others may notice unsteady walking, slowed thinking, blurred concentration or difficulty getting out of bed. These effects can become particularly important when starting gabapentin, increasing its dose, switching to a stronger cannabis product or returning to marijuana after a period of abstinence.

Real-world medicinal-cannabis data support this concern. A study of 275 people receiving medicinal cannabis for chronic non-cancer pain examined adverse effects in relation to their other medications. Patients taking gabapentinoids were approximately 2.4 times more likely to report dizziness than patients who were not taking drugs in that class. The study was observational and included both gabapentin and related medications, so it cannot prove that marijuana plus gabapentin caused the dizziness. Nevertheless, its findings are consistent with additive neurological side effects and demonstrate why medication combinations matter when assessing cannabis tolerability.

The consequences can be more serious for older adults or anyone who already has poor balance. A brief episode of dizziness in a young healthy adult may simply be unpleasant; the same episode in an older person with osteoporosis could lead to a fall and fracture. Gabapentin clearance also tends to fall as kidney function declines with age, meaning an older patient’s exposure to a given dose can increase. Pharmacokinetic research has shown declining renal clearance of gabapentin with increasing age, largely because of age-related reductions in kidney function.

Could Marijuana and Gabapentin Affect Breathing?

Gabapentin has attracted increasing attention because of respiratory depression when it is combined with other central nervous system depressants. In 2019, the FDA required stronger warnings for gabapentin and pregabalin after reviewing reports of serious and sometimes fatal breathing problems. Most cases involved other CNS depressants—particularly opioids—or patients with underlying respiratory impairment. Older adults were also identified as being at increased risk. FDA advises clinicians to monitor for sedation and respiratory depression when gabapentinoids are used alongside other CNS-depressing substances.

Whether marijuana alone meaningfully increases gabapentin-associated respiratory depression has not been established in controlled clinical trials, and it would be inaccurate to treat cannabis as equivalent to an opioid in this respect. The stronger concern arises when several substances are combined. A person taking gabapentin who also uses cannabis, an opioid painkiller, a benzodiazepine such as alprazolam or diazepam, a prescription sleep medication, or substantial amounts of alcohol may accumulate multiple sources of sedation. Extreme sleepiness, inability to awaken normally, confusion, unusually slow or shallow breathing or bluish lips require urgent medical evaluation.

Can Marijuana and Gabapentin Be Used Together for Nerve Pain?

The reason this combination comes up so often is that both treatments are used by patients with neuropathic pain. Gabapentin is an established option in neuropathic-pain guidelines, while cannabinoids have also been studied for chronic pain. A BMJ international clinical practice guideline issued a weak recommendation for trying non-inhaled medical cannabis or cannabinoids in addition to standard treatment when chronic pain remains inadequately controlled. The recommendation was deliberately weak because randomized trials generally show only small improvements in pain, physical function and sleep, accompanied by adverse effects such as dizziness and sedation.

Using cannabis together with gabapentin could theoretically provide additional analgesia through different neurological pathways, and experimental research supports the possibility of additive pain-relieving effects. But there is not enough high-quality human research to define an ideal THC dose, CBD dose or THC ratio specifically for patients taking gabapentin. The University of Kentucky study was designed around THC effects rather than pain relief, and the University of Sydney interaction study was performed in animals. Consequently, patients should not assume that combining the two will automatically provide better pain relief than optimizing one treatment first.

THC and CBD Are Not Necessarily Equivalent

The marijuana-gabapentin interaction is likely to depend heavily on the cannabis product. High-THC marijuana is more likely to produce intoxication, slowed reaction time, altered perception and coordination problems. CBD-dominant preparations may produce less intoxication, but CBD is still pharmacologically active and should not automatically be considered interaction-free. Commercial cannabis products can also contain dramatically different cannabinoid concentrations, so saying someone “uses marijuana” provides much less information than knowing the THC and CBD dose.

Edibles deserve particular caution because their effects can be delayed and prolonged. A person may take gabapentin, consume an edible and decide after an hour that the cannabis dose was too small, only for the THC effect to intensify later. CDC guidance notes that cannabis edibles can have delayed or unpredictable effects and carry a higher risk of excessive consumption than rapidly acting products. When gabapentin is already causing sleepiness, an unexpectedly strong edible can make the combined impairment substantially more difficult to manage.

Kidney Function Matters More Than Many People Realize

Because gabapentin leaves the body primarily through the kidneys, impaired renal function can dramatically change its effects. In a pharmacokinetic study involving people with varying kidney function, declining creatinine clearance led to reduced gabapentin clearance, higher concentrations and a longer elimination half-life. Current prescribing information similarly states that gabapentin’s half-life can increase from approximately 6.5 hours in people with good renal function to around 52 hours when creatinine clearance falls below 30 mL/min.

This means a person who previously tolerated marijuana with gabapentin could become more sensitive if kidney function declines, even if the cannabis product has not changed. Older adults, people with chronic kidney disease and patients receiving dialysis deserve particular caution. Excessive drowsiness or confusion in such patients should not automatically be blamed on marijuana; it can also indicate that the gabapentin dose is too high for current renal function.

What If Gabapentin Is Being Taken for Seizures?

Anyone taking gabapentin as part of seizure treatment should be particularly careful about making medication changes based on cannabis use. Botanical marijuana is not an interchangeable substitute for gabapentin or another prescribed antiseizure medication. Although FDA has approved purified CBD—Epidiolex—for seizures associated with specific rare epilepsy syndromes, that prescription medication is standardized and should not be equated with dispensary CBD oil or THC-rich marijuana.

Gabapentin also should not be abruptly stopped simply because someone wants to use cannabis instead. Current prescribing information warns that rapid discontinuation of gabapentin can increase seizure risk and recommends tapering the medication when it is discontinued. Withdrawal symptoms have also been reported following both short- and long-term treatment. A patient who believes cannabis is allowing them to reduce gabapentin should therefore discuss dose changes with the prescribing clinician rather than adjusting the medication abruptly.

Who Should Be Especially Careful?

Extra caution is reasonable for people who are new to either cannabis or gabapentin, use high-THC products, have poor balance, are older, have impaired kidney or lung function, or already experience significant dizziness from gabapentin. The combination deserves even closer medical review when opioids, benzodiazepines, sleeping pills, sedating antihistamines or substantial alcohol consumption are also involved. In these situations, the important question is not simply whether two drugs technically “interact,” but whether the total medication regimen is producing too much central nervous system impairment.

For patients who use both under medical supervision, minimizing abrupt changes can make side effects easier to identify. Introducing a new high-dose THC product at the same time as increasing gabapentin, for example, makes it difficult to determine which substance caused dizziness or excessive sedation. Driving or operating machinery should be avoided whenever the combination produces impairment. Both the Neurontin prescribing information and CDC cannabis guidance specifically warn about effects relevant to safe driving and coordination.

The Bottom Line

Marijuana and gabapentin can sometimes be used by the same patient, but the combination is not risk-free. There is little evidence of a major metabolic interaction because gabapentin is not appreciably metabolized by the liver and is primarily eliminated unchanged through the kidneys. The more important interaction is functional: both gabapentin and THC can produce dizziness, sleepiness, impaired coordination and slowed psychomotor performance. A small University of Kentucky human study directly examining the combination found overlapping effects and generally stronger THC-sensitive effects when gabapentin and THC were administered together.

For someone who tolerates a stable gabapentin dose and uses a modest amount of cannabis, the combination may not necessarily produce a serious problem. Risk rises when THC doses are high, gabapentin doses are increased, kidney function is impaired, the patient is elderly or other sedating drugs are added. The safest approach is to treat cannabis as another active medication rather than as an unrelated herbal product: tell the prescribing clinician or pharmacist how much THC and CBD are being used, watch for changes in alertness and balance, and avoid changing or abruptly stopping gabapentin without medical guidance.
