Walk into a medical cannabis dispensary and one of the first questions you may encounter is whether you want a THC-dominant, CBD-dominant or “balanced” product. Ratios such as 1:1, 2:1, 10:1 and 20:1 CBD-to-THC have become common shorthand for describing cannabis formulations, particularly among people trying cannabis for chronic pain. The implication is often that choosing the correct ratio unlocks the best analgesic effect. Scientific research, however, paints a more complicated picture. There is no single THC ratio proven to be the best treatment for every type of pain, and the actual milligram dose of each cannabinoid may matter more than the ratio printed on the label.

Current evidence does provide useful clues. Reviews from the U.S. Agency for Healthcare Research and Quality (AHRQ) have found the most consistent evidence of modest pain reduction with products containing either comparable amounts of THC and CBD—roughly a 1:1 ratio—or relatively high amounts of THC. CBD-dominant products have generally produced less convincing analgesic results in randomized trials, although they may be better tolerated and emerging research suggests they could still have a role for certain patients. Most of the stronger clinical evidence involves neuropathic pain, rather than every type of arthritis, back pain, headache or generalized pain.

Understanding What a THC Ratio Actually Means

A THC ratio describes the relative amounts of delta-9-tetrahydrocannabinol and cannabidiol in a cannabis product. A 1:1 product contains approximately equal quantities of THC and CBD. A product labeled 10:1 CBD contains roughly ten times more CBD than THC, while a 10:1 THC product is strongly THC dominant. The ratio alone, however, says nothing about how powerful a dose will be. A capsule containing 2 mg THC plus 2 mg CBD and an edible containing 20 mg THC plus 20 mg CBD are both technically 1:1 products, but they are pharmacologically very different experiences.

That distinction is especially important with THC because its psychoactive and adverse effects are strongly dose dependent. Someone who is sensitive to THC could tolerate 1 mg of THC alongside 1 mg of CBD while experiencing substantial intoxication, dizziness or anxiety from 10 mg of each. Consequently, asking for the “best ratio” is only the first part of the question. A more useful approach is to consider the ratio, the absolute milligram dose, the type of pain being treated, previous THC exposure, other medications and the person’s vulnerability to side effects.

The U.S. Food and Drug Administration has not approved botanical marijuana as a pain treatment and has not established standardized THC dosing ratios for chronic pain. FDA has approved certain cannabinoid-related medications for specific conditions—including purified CBD for particular seizure disorders and synthetic THC-related medicines for other indications—but dispensary cannabis products have not gone through the same FDA approval process for pain. That means recommendations about ratios necessarily rely on clinical trials of individual cannabinoid preparations, systematic reviews and expert guidelines rather than an FDA-approved cannabis dosing standard.

The Strongest Evidence Currently Favors Balanced THC and CBD

Among the formulations studied most extensively for chronic pain is nabiximols, an oromucosal cannabis extract containing THC and CBD at approximately a 1:1 ratio. Although nabiximols is not FDA-approved in the United States, it has been used in clinical research and medical treatment elsewhere. AHRQ’s continuing review of cannabis treatments for chronic pain found that extracted cannabis products containing comparable amounts of THC and CBD produced a small improvement in pain severity and overall function compared with placebo, with much of the evidence coming from neuropathic-pain trials.

One randomized, double-blind study led by researchers at the University of Liverpool examined a THC cannabis spray in 125 people with peripheral neuropathic pain characterized by allodynia—a painful response to normally non-painful stimulation. Participants continued their existing pain medications while adding either the cannabinoid spray or placebo. The reduction in pain scores was significantly greater with THC/CBD treatment. Another randomized crossover trial involving difficult-to-treat brachial plexus nerve-injury pain compared an approximately 1:1 THC extract, a predominantly THC extract and placebo. Both cannabinoid formulations produced statistically significant improvements in several pain and sleep measures, although the magnitude of improvement was not large enough to meet the trial’s prespecified primary target.

These studies help explain why 1:1 has become one of the most commonly discussed starting ratios for cannabinoid-based pain treatment. The evidence does not prove that 1:1 is universally superior to 2:1 or 4:1, nor does it show that every person needs equal quantities of both cannabinoids. Rather, balanced THC/CBD formulations happen to be among the cannabis preparations for which controlled pain data are relatively well developed.

High-THC Products May Reduce Pain—but Side Effects Increase

THC appears to be responsible for a substantial portion of cannabis’s direct analgesic activity. AHRQ’s evidence review found that synthetic or purified high-THC products were also associated with small improvements in pain severity, again primarily in studies of neuropathic pain. This is an important finding because it challenges the popular assumption that large quantities of CBD are necessary for cannabis to relieve pain. In some controlled studies, THC-dominant preparations performed at least as well as cannabinoid combinations from the standpoint of analgesia.

The problem is tolerability. Higher-THC formulations are more likely to produce dizziness, sedation, impaired attention, altered coordination, anxiety and intoxication. In AHRQ’s assessment, high-THC products substantially increased the likelihood of dizziness, while THC-containing formulations also increased sedation and nausea. The BMJ’s international clinical practice guideline consequently gives only a weak recommendation for a trial of non-inhaled medical cannabis or cannabinoids in chronic pain when standard care has not provided adequate relief. The recommendation is weak because the average improvements in pain, sleep and function are small and must be balanced against adverse effects.

For that reason, a ratio such as 5:1 or 10:1 THC should not automatically be interpreted as a “stronger pain medicine” that is therefore better. Increasing THC may increase analgesia in some patients, but the therapeutic window can narrow quickly. The useful dose is the amount that improves pain or function without producing unacceptable impairment. Someone who achieves relief with 2.5 mg THC has little reason to take 15 mg merely because a more THC-dominant product is available.

Is CBD-Dominant Cannabis Better for Pain?

CBD-dominant products are appealing because CBD does not produce the characteristic THC high. Ratios such as 10:1, 20:1 or even higher CBD-to-THC are therefore frequently recommended to people who want symptom relief while remaining clear-headed. Yet the clinical evidence for CBD as a stand-alone pain treatment is less impressive than popular marketing sometimes suggests. In AHRQ’s review, purified or synthetic CBD alone did not significantly improve chronic pain intensity or function compared with placebo. Products containing relatively low THC compared with CBD also failed to demonstrate consistent improvements in the randomized studies available.

That does not mean CBD has no potential role in pain management. Clinical research is still developing, and results can differ according to the pain condition, cannabinoid formulation and outcome being measured. A 2026 study involving researchers from the University at Buffalo and University of Michigan examined different cannabinoid formulations in adults with fibromyalgia, rheumatoid arthritis and knee or hip osteoarthritis. Participants received one of three cannabinoid formulations, including a 1:1 CBD/THC capsule containing 12.5 mg of each cannabinoid, a formulation containing several non-intoxicating acidic and minor cannabinoids, or a CBD/CBDa preparation. Participants completing the 12-week study reported improvements across many symptom measures, and most differences were not clearly dependent on which cannabinoid combination they received. Because the study lacked a placebo group and relied on per-protocol analysis, it cannot establish that any particular ratio caused the improvements, but it illustrates why researchers are continuing to investigate non-intoxicating cannabinoid combinations.

CBD-dominant formulations may also be useful when THC tolerability is the limiting factor rather than maximum analgesic potency. A 2026 German real-world study involving adults 65 and older with chronic or refractory pain found CBD-dominant full-spectrum extracts were associated with fewer adverse reactions and better multidimensional outcomes than dronabinol, a THC medication. Because this was an observational registry analysis rather than a randomized trial, it cannot prove CBD dominance is superior. It does, however, reinforce the practical reality that the most effective ratio is useless if a patient cannot tolerate it.

Best THC Ratio for Neuropathic Pain

If any category currently has a reasonable argument for a balanced THC formulation, it is neuropathic pain. Neuropathic pain originates from damaged or dysfunctional nerves and can manifest as burning, electrical sensations, tingling, shooting pain or painful sensitivity to touch. Conditions include diabetic peripheral neuropathy, postherpetic neuralgia, multiple-sclerosis-related central pain and certain nerve injuries. A substantial portion of cannabis pain trials reviewed by AHRQ have involved neuropathic conditions, and comparable THC/CBD formulations have repeatedly demonstrated small analgesic effects.

A reasonable interpretation of the evidence is therefore that approximately 1:1 THC has stronger clinical support for neuropathic pain than CBD alone, but this should not be mistaken for proof that exactly one milligram of CBD must accompany every milligram of THC. Individual randomized trials have produced mixed findings. One study comparing oral CBD, THC, their combination and placebo in peripheral neuropathic pain did not establish a clear universally effective cannabinoid strategy despite allowing substantial dose titration. This inconsistency is one reason major guidelines remain cautious about cannabinoid treatment rather than declaring a particular ratio the standard of care.

Best THC Ratio for Arthritis and Inflammatory Pain

The evidence is much thinner for selecting a precise cannabis ratio for osteoarthritis, rheumatoid arthritis and other musculoskeletal pain. Although many medical cannabis patients use cannabinoids for joint pain, the strongest randomized evidence supporting cannabis for chronic pain overall cannot automatically be translated into an ideal arthritis formulation. AHRQ specifically notes that the available cannabinoid literature is dominated by neuropathic-pain research and that evidence for whole-plant cannabis and many other formulations remains insufficient.

For someone with arthritis who is trying cannabinoid therapy under medical supervision, the evidence therefore favors a titration strategy rather than choosing an aggressive fixed ratio from the beginning. A CBD-predominant formulation may be easier to tolerate during daytime activities, while small amounts of THC can be added if pain remains inadequately controlled. A balanced formulation may be reasonable for someone who already tolerates THC, particularly when pain disrupts sleep. What the research does not currently justify is claiming that a 20:1, 4:1 or 1:1 product has been proven specifically optimal for osteoarthritis or rheumatoid arthritis.

Fibromyalgia May Be Even More Individualized

Fibromyalgia is sometimes described as nociplastic pain because altered nervous-system pain processing appears to play an important role. Cannabis is frequently used by people with fibromyalgia, but determining an optimal THC ratio is particularly difficult because symptoms extend beyond pain to include sleep disruption, fatigue, cognitive complaints and mood symptoms. A formulation that improves nighttime pain while causing next-day cognitive impairment may represent a poor overall result even if pain scores fall.

The 2026 University at Buffalo/University of Michigan-associated study is noteworthy because it included patients with fibromyalgia and directly compared several cannabinoid compositions, including equal CBD and THC. Most outcome improvements did not differ dramatically between products, and the investigators concluded that different cannabinoid combinations may have potential across several classes of chronic pain while non-intoxicating cannabinoids could be useful where intoxication is undesirable. These findings are promising, but they reinforce rather than eliminate the need for larger placebo-controlled trials that directly compare ratios.

The Dose May Matter More Than the Ratio

One of the easiest mistakes when choosing medical cannabis for pain is focusing on ratios while ignoring milligrams. Consider three products: 1 mg THC + 1 mg CBD, 5 mg THC + 5 mg CBD, and 20 mg THC + 20 mg CBD. Every one is a 1:1 formulation, yet their effects can be dramatically different. The first may produce little noticeable intoxication for many adults, while the third can be an extremely strong THC exposure for someone without tolerance.

Expert consensus recommendations for chronic-pain cannabis therefore emphasize gradual titration. A published modified-Delphi consensus involving clinicians from several countries proposed a routine approach beginning with a CBD-predominant product at 5 mg CBD twice daily, increasing CBD as needed before introducing THC at approximately 2.5 mg and titrating slowly. For frail patients, older adults or people with complicated medical conditions, the conservative protocol begins at only 5 mg CBD once daily and introduces THC at approximately 1 mg per day if needed. A faster protocol intended for selected patients begins with a balanced THC product containing roughly 2.5–5 mg of each cannabinoid. These are expert-consensus recommendations rather than FDA-approved dosing instructions, but they illustrate the principle that cannabis treatment is usually better approached by milligram titration than by ratio shopping alone.

Daytime and Nighttime Ratios May Be Different

For some chronic-pain patients, the “best” ratio may change over the course of a day. THC-related sedation and intoxication can interfere with driving, employment, caregiving and concentration. A CBD-dominant formulation with very little THC may therefore be preferable during daytime hours even when it provides somewhat less analgesia. At night, a patient who tolerates THC may choose a more balanced formulation because mild sedation is less disruptive and sleep improvement may itself be valuable.

This strategy remains more clinical practice logic than proven ratio science. Controlled trials have not established that everyone should use CBD during the day and 1:1 cannabis at night. Still, separating daytime functionality from nighttime symptom management illustrates why attempting to identify one universal ratio can be misleading. Chronic pain is not simply a number on a scale; treatment also needs to consider sleep, cognition, mobility and the ability to function normally.

Does CBD Reduce the Side Effects of THC?

One popular explanation for balanced cannabis is that CBD supposedly “cancels out” THC intoxication. The reality is considerably more complicated. CBD and THC interact pharmacologically, but CBD does not reliably neutralize THC at the doses and ratios typically found in cannabis products. Depending on dose, timing and route of administration, CBD can modify THC’s effects in different ways. Consumers should therefore not assume that 10 mg THC becomes equivalent to a mild 5 mg exposure simply because the product also contains 10 mg CBD.

From a pain-treatment perspective, the reason to consider a 1:1 formulation is therefore not that CBD makes substantial quantities of THC harmless. It is that comparable THC/CBD preparations have accumulated a meaningful body of randomized clinical evidence, particularly in neuropathic pain. Side effects still occur. In the AHRQ analysis, balanced extracted THC/CBD products substantially increased dizziness and sedation compared with placebo and also increased nausea.

A Practical Evidence-Based Starting Point

For someone asking purely what current clinical research suggests, the best-supported general answer is approximately 1:1 THC for chronic neuropathic pain when THC is tolerated, used at the lowest effective cannabinoid doses. High-THC products may also reduce pain, but their adverse-effect burden is generally greater. CBD-dominant products provide a lower-intoxication option and may be appropriate for people especially sensitive to THC, but randomized evidence for CBD alone as a direct analgesic is considerably weaker.

That should not be converted into a universal recommendation that everyone with pain buy a 1:1 product. Older adults, cannabis-naive patients, people prone to dizziness or anxiety, those with cardiovascular disease and anyone taking medications that interact with cannabinoids may need much more conservative THC exposure. Likewise, someone who has previously tolerated THC may respond differently. The BMJ guideline’s weak recommendation for cannabinoid therapy reflects precisely this close balance between small potential benefits and potentially troublesome side effects.

The Bottom Line

There is no scientifically established single best THC ratio for pain, but the evidence is not completely neutral. Comparable THC and CBD formulations—most commonly represented by approximately a 1:1 ratio—currently have some of the best clinical evidence for modest relief of chronic neuropathic pain. THC-dominant products may also reduce pain but generally produce more intoxication, dizziness and other adverse effects. CBD-dominant products are often easier to tolerate and may have roles in particular patients, yet CBD alone has not consistently reduced chronic pain in randomized trials.

The most important lesson is that ratio and dose must be considered together. A carefully titrated 1:1 product containing 1–2.5 mg of THC is fundamentally different from a 1:1 edible containing 20 mg THC. For medical use, the goal should not be to find the strongest cannabis product or the ratio with the most impressive label. It should be to identify the lowest cannabinoid exposure that produces meaningful improvement in pain, sleep or function without causing unacceptable impairment. As researchers conduct more head-to-head trials of specific cannabinoid combinations, recommendations may eventually become more precise. For now, the evidence favors individualized dosing—and much more caution than the simple phrase “best THC ratio” might suggest.
